Trientine: All about the medication

Trientine is an orally formulated copper chelating agent used to treat Wilson’s disease, also known as hepatolenticular degeneration in those who can not use penicillamine. Trientine and penicillamine are incompatible; Trientine needs to be used when penicillamine is no longer possible because of intolerable or mortal side effects.


Trientine generic is not for patients with RA, cystinuria, or biliary cirrhosis. Patients must remain under proper medical supervision while on Trientine, and female patients should be precisely monitored for iron deficiency anemia.

Trientine is one of several copper-chelating agents that lower blood and tissue copper levels and, when given for a more extended period, prevent copper accumulation and injury in Wilson disease (WD). D-penicillamine and dimercaprol are other existing copper-chelating agents.


Overview of Disease:

Wilson’s disease is an inherited disorder; excess copper accumulates initially in the liver but also in the kidneys, brain, and cornea. In disease, copper is not eliminated precisely and instead gets, possibly, to deadly levels.


Wilson’s disease (WD) occurred due to mutations of the gene ATP7B on chromosome 13, which encodes a copper-transporting ATPase in the liver. The condition affects around 1 in 30,000 individuals and is commonly present in childhood or adolescence with neurologic syndromes, signs of advanced liver disease, and hemolytic anemia.


Dosage and Routes of Administration:

Trientine is supplied in capsules of 250 mg generically and 333 mg under the brand name Trilawil. The proposed dose is 750 to 1250 mg for adult patients and 500 to 750 mg for children, administered in 2-4 divided doses daily initially, which can be upgraded to a maximum of 2000 mg daily for adult patients and 1500 mg daily for children.


Trientine dihydrochloride 333 mg capsules must be taken on an empty stomach, at least 1 hour before meals or a couple of hours after taking meals, and at least 1 hour apart from any other drug, food, or milk. The capsules must be gulped whole with water without opening or chewing them.


Possible Side Effects of Trientine:

Side effects of Trientine are generally minimal and may include nausea, headache, myalgias, arthralgias, renal dysfunction, anorexia, diarrhea, and rash. Uncommon but potentially critical side effects include hypersensitivity reactions, pancytopenia, and lupus-like syndromes.

In conclusion, trientine salt is promising in controlling Wilson’s disease (WD). Side effects are infrequent. In Wilson’s disease (WD), adherence to therapeutic medicine is a significant factor for treatment success. Trientine is not uniformly accessible or available in all countries, but Trientine price in India is reasonable.
