Sofosbuvir was a breakthrough new medicine for the treatment of patients with chronic hepatitis C. Sofosbuvir has so many of ideal properties, including once daily dosing, no meal restrictions, few side effects, minimal drug-drug interactions, high genetic barrier to resistance, and relatively better safety and efficacy in patients with advanced liver disease. The use of sofosbuvir together with ribavirin was the first FDA-approved oral therapy for hepatitis C. Of note, the activity against genotype-3 appears less than with genotype-2 and treatment of genotype-3 infection needs a longer all-oral course of treatment than with genotype-2. Sofosbuvir is primarily recommended now in fixed-dose combinations.
Indications: The following are the medical use of sofosbuvir. Sofosbuvir is used for treatment of patients with chronic HCV
● Genotype-1/4: sofosbuvir + peginterferon-alfa + ribavirin for 12 weeks.
● Genotype-2: sofosbuvir + ribavirin for the 12 weeks.
● Genotype-3: sofosbuvir + ribavirin for 24 weeks
In order to treat patients with HCV-HIV-1 coinfection: the recommendations are the identical as listed above.
For patients with genotype-1 HCV who are not eligible to receive interferon: sofosbuvir along with ribavirin for 24 weeks can be considered.
For patients with the hepatitis C virus and HCC awaiting liver transplantation: the sofosbuvir can be used along with ribavirin for up to 48 weeks or until liver transplantation, whichever occurs first.
The sofosbuvir velpatasvir combination is used alone or with ribavirin in order to treat chronic hepatitis C (an ongoing viral infection, responsible for damaging the liver) in adults and children aged 6 years and older or who weigh at least 37 lbs (17 kg).
Dosing: Sofosbuvir comes as a 400 mg tablet. The standard dosage of sofosbuvir is 400 mg, which should be administered orally once daily, with or without food. The dose of sofosbuvir 400 mg should be used, regardless of the patient's genotype and prior hepatitis C treatment experience. No dose adjustment is required for mild-to-moderate renal impairment or with mild, moderate, or severe hepatic impairment. Sofosbuvir also comes as a fixed-dose combination medicine (ledipasvir 90 mg and sofosbuvir 400 mg) administered once daily.
Administration: Administer single tablet of Sofosbuvir one time each day only, either with or without food. Administer Sofosbuvir exactly as your health specialist tells you to administer it. Avoid changing your dosage unless your health specialist tells you to. Do not discontinue or stop taking Sofosbuvir without first talking with your health specialist. If you think there is a reason to discontinue or stop taking Sofosbuvir, interact with your health specialist before doing so. In case you miss a dose of Sofosbuvir, administer the missed dose as promptly as you remember the same day. Do not administer more than 1 tablet (400 mg) of Sofosbuvir in a day. Administer your next dose of Sofosbuvir at your regular time the next day. In case you administer an excess dose of Sofosbuvir, call your health specialist or go to the nearest hospital emergency room right away.
Discontinuation of Dosing: In case the other respective agents used along with Sofosbuvir are permanently interrupted or discontinued, the use of Sofosbuvir should also be interrupted or discontinued.
